The Residency & Fellowship Window: Why Physicians Should Secure Disability Coverage During Training
The best time for a physician to purchase individual disability insurance isn't after they've built a practice — it's during residency or fellowship, when premiums are lowest and health is most likely to qualify for preferred rates.
Medical residents and fellows are often laser-focused on completing training, managing student debt, and preparing for their first attending position. Disability insurance tends to fall low on the priority list — but ironically, the training years represent the single best window to secure coverage.
Here's why: disability insurance premiums are based on age, health, occupation, and income at the time of application. During residency, physicians are at their youngest and typically their healthiest. Most carriers offer special programs for residents and fellows that allow them to purchase coverage based on their projected future income — not their current trainee salary.
These 'future increase' or 'benefit update' riders allow physicians to increase their coverage as their income grows, without additional medical underwriting. A resident who locks in a $5,000/month benefit with a future increase option can later increase coverage to $15,000 or $20,000/month as an attending — regardless of any health changes that occur in the interim.
Waiting until after training introduces risk. A new diagnosis — even something as common as anxiety, back pain, or elevated cholesterol — can result in exclusion riders, higher premiums, or outright denial. Every year of delay is a year of exposure to conditions that could limit future insurability.
Additionally, some of the most physician-friendly carriers and policy features are only available through training-specific programs. Once a physician leaves the training environment, they may lose access to certain discounts, simplified underwriting, and unisex pricing that benefit female physicians in particular.
